Persistent hypoglycemia in patients without diabetes poses a significant diagnostic challenge in clinical practice. Although the most common causes include insulinomas, nesidioblastosis is a rare entity that should also be considered. This condition is characterized by an abnormal proliferation of pancreatic beta cells with inappropriate hyperinsulinemia. The case presented describes a young patient with recurrent hypoglycemia since adolescence, in whom the diagnostic evaluation required the use of advanced functional imaging techniques to confirm the presence of nesidioblastosis. At the time of diagnosis, specific treatment with Diazoxide was initiated with rapid improvement of the clinical picture. Timely diagnosis and treatment of this entity was essential to avoid serious and potentially irreversible complications.
